AB-41 – Catered Event Certificate and Notice
What caterer’s license holders need to know about updated forms and submission requirements
Maryland updated the Catered Event Certificate and Notice form used by statewide caterer’s license holders.
The revised form includes additional required information and must be used for all future submissions.

What you need to know
- A revised catered event form must be used
- The updated form requires additional contact information
- Old forms may be used temporarily if completed correctly
- Forms must be submitted on time and fully complete
Who This Applies To
This guidance applies to:
- Statewide caterer’s license (SCAT) holders
- County liquor control boards
What Changed
The Catered Event Certificate and Notice form was revised to include:
- The phone number of the person contracting the event
This information is now required on all forms.
Using the Updated Form
You should:
- Begin using the revised form immediately
- Ensure all required fields are completed before submission
Using the updated form helps avoid delays or compliance issues.
Using Existing (Old) Forms
If you still have older versions of the form:
- You may continue using them temporarily
- You must manually include:
- The phone number of the person contracting the event
Incomplete forms will not meet submission requirements.
Submission Requirements
When submitting the form:
- The pink copy must be filed with the local Board of License Commissioners
- Submission must be made at least 5 days before the event
Forms must be:
- Complete
- Accurate
- Submitted on time
Important Notes
- Incomplete forms will not satisfy filing requirements
- All required information must be included before distribution
- Using the updated form is recommended as soon as possible
